Critical Insights, Inc. is the sole owner of the information collected
on the www.criticalisights.com
and www.cisurveys.com Websites.
Critical Insights collects personally identifiable information from
our users at several different points on our Web site(s).

Us
We do not collect personally identifiable information unless you choose
to provide it to us. If you provide us with personally identifiable
information, for example, by sending an e-mail or by filling out a form
and submitting it through our Web site, we use that information to respond
to your message and to help us provide you with the information and
services that you request. All uses of that information are described
on the Web page containing the form.
Submitting voluntary information constitutes your consent to the use
of the information for the stated purpose. When you click the "Submit"
button on any of the Web forms found on our sites, you are indicating
your voluntary consent to use of the information you submit for the
purpose stated.
Surveys
If you
participate in one of our surveys, we may request certain personally
identifiable information. Participation in these surveys is completely
voluntary and you therefore have a choice whether or not to disclose
this information. The requested information typically includes contact
information (such as name and address), and demographic information
(such as zip code, income level, gender, etc.).
We use
this information as a "unit of analysis" (ie: to segment data).
In any reports prepared, no information will be included that will make
it possible to identify a particular subject. Research records will
be kept in secure files and only researchers will have access to the
records. The data will be destroyed after three years.
We DO
NOT use third party service providers to conduct our surveys. We will
not share the personally identifiable information you provide through
a survey with other third parties.

Log Files
As is true of most Web sites, we gather certain information automatically
and store it in log files. This information includes internet protocol
(IP) addresses, browser type, internet service provider (ISP), referring/exit
pages, operating system, date/time stamp, and clickstream data.
We use this information, which does not identify individual users, to
analyze trends, to administer the site, to track users' movements around
the site and to gather demographic information about our user base as
a whole.
We DO NOT link this automatically-collected data to personally identifiable
information. We DO NOT use a third-party tracking service [called XXXX]
that uses cookies [log files] to track non-personally identifiable information
about visitors to our site in the aggregate [state what information

Cookies/Sessions
A cookie is a small text file that is stored on a user's computer for
record-keeping purposes. We use both session ID cookies and/or persistent
cookies. [We use session cookies to make it easier for you to navigate
our site and for us to track your progress during a survey. A session
ID cookie expires when you close your browser. A persistent cookie remains
on your hard drive for an extended period of time. You can remove persistent

We set cookies to track your progress through multi-page surveys. We
set cookies to close a survey after you have finished. If you reject
cookies, you may still use our site, but your ability to take some surveys
may be limited.
Clear Gifs (Web Beacons/Web Bugs)
From time to time we employ a software technology called clear gifs
(a.k.a. Web Beacons/Web Bugs), that help us better manage content on
our site by informing us what content is effective. Clear gifs are tiny
graphics with a unique identifier, similar in function to cookies, and
are used to track the online movements of Web users. In contrast to
cookies, which are stored on a user's computer hard drive, clear gifs
are embedded invisibly on Web pages and are about the size of the period
at the end of this sentence. We DO NOT tie information gathered by clear
gifs to our customers' personally identifiable information.
We use clear gifs in our HTML-based emails to let us know which emails
have been opened by recipients. This allows us to gauge the effectiveness
of certain communications and the effectiveness of our marketing campaigns.

collect sensitive information from our Website (such as credit card

to time sensitive information about a user may be provided to Critical
Insights via a client who has asked us to conduct a survey. We store
such sensitive information about survey participants in our databases.
We follow
generally accepted industry standards to protect the personal information
stores on our servers and submitted to us, both during transmission
and once we receive it. No method of transmission over the Internet,
or method of electronic storage, is 100% secure, however. Therefore,
while we strive to use commercially acceptable means to protect your
personal information, we cannot guarantee its absolute security.

In the event Critical Insights, Inc. goes through a business transition,
